If advanced osteoarthritis of the knee joint (wear and tear disease) causes considerable suffering and conservative treatment no longer brings any improvement, it may be necessary to replace the joint. In knee arthroplasty, a distinction is made between partial and total joint replacement. In partial joint replacement, only the outer or inner part between the upper and lower leg is replaced with a so-called sled endoprosthesis. In rarer cases, a partial replacement of the joint between the kneecap and thigh is also performed. In a complete surface replacement (total endoprosthesis), the surface of the upper and lower leg is replaced by the artificial joint.
The installation of the knee endoprosthesis leads to a significant improvement in symptoms, but the clinical results of conventional installation of the endoprosthesis are not as good as those of patients with a hip endoprosthesis.Professor Gollwitzer and Professor Weber have therefore been fitting total endoprostheses with anatomical alignment for many years. By fitting the prosthesis, the individual anatomy is restored as it was before the osteoarthritis. This technique is called kinematic alignment and leads to a stable and harmonious movement of the knee joint without instability or overstressing. Before the kinematic alignment, the axes are measured on an X-ray of the entire leg so that they can then be reconstructed.
At the ATOS Private Clinic in Munich, partial and total endoprostheses are fitted using robot-assisted surgery (MAKOplasty). Before the operation, a model of the leg is created using 3D computer tomography, on which the knee endoprosthesis is planned before the operation. Once the planning has been completed, robotics enables the planned implant positioning to be realised with high precision during the operation, taking into account the ligament tension of the knee joint. The knee joint is adapted to the individual anatomy and fitted.The robot supports the surgeon during the procedure, who is in control of the operation at all times and also performs it himself.
